What is Breast Enlargement Surgery?

Breast enlargement, or breast augmentation, in its simplest terms, is a surgical operation to enlarge or reshape the breasts. This procedure is typically for women who want to improve their bodily proportions and boost their self-esteem. It is not only the most sought after breast surgery in the UK, but the most sought after cosmetic surgery overall.

During the breast enlargement surgery, the surgeon will create a small incision under the breast, then insert a breast implant, made from high-quality silicone or saline, or carry out a fat graft in order to change the size, shape, fulness, or sometimes texture of the breast.

This procedure is also known as breast enhancement, and is often socially referred to as a ‘boob job’.

Breast enlargement surgery is typically sought out by women who desire to enhance the shape or size of their breasts. They may have a naturally small bust, or asymmetrical breasts with one breast sized or shaped differently than the other.

In some cases, this procedure may be chosen by women who have undergone a mastectomy as part of cancer treatment and want enlargement surgery as part of the breast reconstruction process.


At Cadogan Clinic, we take two approaches to breast augmentation in London:

  • Implant augmentation: This approach places silicone or saline implants behind the breast tissue in front of the chest muscle, behind the muscle, or a combination of both using a split muscle technique. We only ever use the very best quality implants, and have never nor will ever use PIP implants at our clinic.
  • Fat transfer augmentation: In this method, the surgeon will use liposuction to collect fat from another part of your body, such as the thighs or buttocks, and use fat grafting techniques to increase the volume of the breast. This procedure is also known as natural breast augmentation.

Your surgeon will discuss in more detail which enlargement procedure or implant type is the most suitable for you.

Breast Augmentation & Implant Options

There are several options to consider for breast enhancement or augmentation surgery. Your surgeon will discuss these with you at your consultation.

Implant Size

Breast implants are available in a variety of sizes. There are three different shapes available: round, teardrop, and conical, with four different profiles ranging from 'low' to 'extra high'.This grading refers to where the implant's edge touches the breast. A low-profile breast implant will give a softer, more natural result. A sharper edge on an extremely high profile results in a more dramatic, but more 'false' appearance.

As implants are given by volume in millilitres or weight in grammes, you cannot specify a cup size. Because of many factors such as the amount of existing breast tissue and the amount of accessible skin, our surgeon cannot guarantee your ultimate cup size.

Implant Shape

Breast implants come in three different forms.

Round breast implants are spherical and provide a fuller appearance at the top of the breast. If you want to remedy breast drooping, round implants are a wonderful alternative.

Teardrop breast implants get their name from their shape, which has a higher volume at the bottom of the implant. When compared to round breast implants, teardrop breast implants produce a more natural-looking result.

Conical breast implants are the ideal option for women who wish to improve the contour of their breasts without having to undergo a breast lift (mastopexy). London breast augmentation produces more long-term results than a breast lift.

All of these are available in four different profiles ranging from 'low' to 'extra high'.

Asymmetry

Breast asymmetry is extremely common, afflicting more than half of all women. Some people are unconcerned by this, but it can make others feel self-conscious. Asymmetric breasts develop when one breast is larger, smaller, or in a different location than the other. Hormonal changes, puberty, and trauma may all be at blame.

A rare condition known as juvenile hypertrophy of the breast can cause one breast to develop much larger than the other. Although some small disparities may persist after surgery, breast asymmetry can be rectified as part of the treatment.

Breast asymmetry is usually not a cause for concern; however, if there is a significant variance in asymmetry or the density of one breast abruptly changes, this could be an indication of cancer. If you observe any drastic or rapid changes in your breasts, you should consult your doctor.

Implant Positioning

When it comes to breast implant positioning, there are two alternatives. Implants can be put either in front of or behind the pectoralis major (chest wall muscle).

Placing the breast implants in front of the muscle, behind the breast, is the most straightforward of the two options, and is highly effective at correcting slightly drooping breasts.

Inserting breast implants behind the muscle is the ideal option for slender patients and those with very little breast tissue, since the muscle provides cover to assist disguise the upper portion of the implant. This decreases the possibility of seeing or feeling the implant in place. This place, however, can often cause substantial discomfort to patients.

At Cadogan Clinic, we also offer a combination of both placements, using a split muscle technique.

Saline or Silicone

Boob job UK cases are typically completed using one of two different types of implants.


Saline: These breast implants are made of sterile salt water. When the implant is placed in the body before being filled with water, it is empty. This means that the incisions needed to install the implants are smaller than for a silicone implant.

Saline implants are sometimes more noticeable under the skin than silicone implants. Some women claim to be able to feel the ‘water sloshing’ around in them. Though saline implants are usually less expensive than silicone implants.

In the event the implant should rupture, the body will absorb this liquid with no risk to health. You will need further surgery to replace the deflated implant.


Silicone: Silicone is the most often used implant material in the UK and is the most popular choice for breast implant material. Silicone is non-toxic, long-lasting, and has a more natural appearance and feel. Silicone implants are particularly suitable for slim women with little breast tissue.

Type of Incision

Our expert surgeons will make the incisions as discreetly as possible under the breast. There are various incisions which can be made to put breast implants in place.

The inferior mammary fold, or IMF, is the most common and safest. This fold occurs naturally and can be found where the wire in an underwire bra would be. Other incisions include the periareolar, which is done around the nipple, and the axillary, which is made in the patient's armpit.

Although our surgeons strive to keep scarring to a minimum, all breast enhancement patients will have some scarring. Scars vary depending on the size of the implants.

Types of Stitches/Sutures

All sutures are placed below the skin and are naturally dissolved by the body over a few weeks. Dissolvable stitches are completely safe and are utilised by numerous medical professionals in many healthcare settings. The use of dissolvable stitches means you do not need to make a follow-up appointment for removal.

Often our surgeons will opt to seal the wound with special tissue glue. This helps to reduce the danger of superficial wound infection caused by the bacteria that everyone has on their skin. The adhesive will then begin to flake off one or two weeks after it is applied.

Who is suitable for breast enlargement?

To be a good fit for a breast enlargement surgery, it’s important you fit the following criteria.

  • an adult with fully-developed breasts,
  • realistic about the expectations of what can be achieved by surgery,
  • physically and psychologically fit and healthy,

01.The consultation

Before you undergo your breast enlargement in London procedure you will have a consultation and a pre-operative assessment at the clinic to ensure your suitability. During this assessment, the surgeon will take several measurements of your breasts and chest area whilst documenting your skin tissue quality and laxity, as well as the nipple position.

Your surgeon will then discuss a selection of implant shapes (such as round, teardrop, or conical), sizes, and surface textures that will then be produced to fit your desired aesthetic outcome. The incision site will also be decided upon.

A comprehensive discussion will take place informing you of the risks and complications of the procedure, alongside a conversation about your expected post-procedure and recovery phase. Your surgeon will also record your previous medical history, including any previous medications or allergies, and will complete an assessment of your fitness for surgery. Finally, standard clinical photography will be taken, with the potential for a visual outcome of your procedure using Crysalix 4D imaging technology.

After your assessment, if your surgeon believes you will be a good candidate for surgery, you will be invited to consider whether you would like to continue with the surgery or otherwise with a two-week 'cooling off' period. During this period, you are welcome to discuss the surgery with your surgeon as many times as you would like.

02.Leading up to procedure

If you decide to proceed with your surgery, your next visit will be the day of your procedure. We recommend you make the following changes before the day of your surgery:

  • In the 6 weeks running up to your surgery, we highly recommend you cease smoking as this can cause a higher risk of infection, wound breakdown, and ultimately the need for implant removal.
  • In the 7 days before your surgery, you should discontinue the use of Aspirin or any other medications that contain Aspirin.
  • In the 6 hours before your surgery, you must refrain from consuming any food or drink. You can take small sips of clear fluid, such as water, black tea, or black coffee, until 2 hours before admission.

03.On the day

On the day of your surgery, a nurse will take your blood pressure and relevant vitals. You will then meet with your anaesthetist and surgeon who will make the final mark-ups.

Breast enlargement is a relatively straightforward procedure which takes an hour under general anaesthetic. An incision will be made in a discreet place, usually on the inframammary fold under the breast or along the areolar edge (periocular) or under the armpit (axillary). The implant will be inserted in front of, behind or in between the chest muscle as decided by your surgeon and your expected outcomes. The incision is sutured afterwards, typically leaving a scar of 4 to 5 inches or 10 to 13 centimetres.

04.Treatment recovery

After your procedure, you will recover for one or two hours in our specialist recovery rooms. Once our team of nurses have ensured your recovery is complete, and that you are safe to leave the clinic, you can leave accompanied by a friend or family member.

You should expect any pain to subside within 1 to 4 days after surgery, and many patients are pain-free by the end of the first week. Cadogan Clinic recommends you take 1 week off work for recovery, with very minimal movement in the first 2 weeks.

It’s essential to wear a post-surgical garment for 6 weeks after your procedure to reduce the risk of implant rotation and ensure long-term scar healing. We also suggest you avoid any strenuous upper body exercise for this time.

After 1 week of recovery, we suggest you visit the clinic so we can ensure your incision sites are healing properly. You are also given the opportunity to meet with our on-site aestheticians to discuss treatment to support the scarring and healing process. After 6 weeks, we recommend a final post-procedure check-up.

You will have access to our dedicated on-call nursing team 24 hours a day, 7 days a week after your surgery. Our team of specialists are there to field any questions you may have, as well as support your pain and comfort control.

There are various types of implants used in breast enhancement surgery. The primary difference is between saline and silicone implants. There are also textured and smooth implants.

The external appearance of these different types of implants is broadly similar regardless of the material. Due to silicone's favoured features as the safest, most inert of prosthetic materials and their somewhat more natural feel post-operation, silicone implants remain the most often utilised implant material in the UK.

Breast augmentation and breast lift (mastopexy) can be performed concurrently.

Breast lift surgery is used to elevate and/or restructure breasts that droop low over the chest and have the majority of their volume at the base. It is an effective method for restoring firmer, younger-looking, and more voluminous breasts.

Breast augmentation with implants aims to increase the size of the breasts. If you are thinking about combining the two operations, keep in mind that a breast lift requires more recovery time than breast enlargement alone.

Different surgeons have different protocols, but it is common for a special garment to be worn in the immediate post-operative phase. The wounds are generally closed with stitches under the surface, which then dissolve over a few weeks. A strip dressing or reinforcing paper strips may be applied over or along the wound. Gauze and stretchy Mefix paper dressings may also be used to cover the wound and support the breasts.

After that, a compression garment is usually worn to support the breasts and protect the wounds. Although some surgeons do not use them at all and recommend a sports bra instead, it is most common to wear a lycra compression bra made specifically for breast surgery of which there are many types. The experts will select the best one for you and your surgery. There is also one other lycra dressing, occasionally required, and that is a stretchy strap worn across the upper breast to hold the implant low.

The sports bras or lycra garments are going to be worn day and night for at least three weeks, and then during the day for an extended period of time, particularly during exercise or sporting activity.

The extent of the scarring will depend upon the types of incision used in the surgery. Although scarring is kept to a minimum, you should expect some scarring following a breast enlargement procedure. Scars can be expected where the underwire on a bra would sit, around the nipple area or in the armpit.

Scarring will be at its most noticeable during the first few months following surgery. Scars will be raised and a different colour to your skin. They will eventually flatten and fade. The scars may not take on their final appearance until a full 18 months after surgery.

The recovery time for breast enlargement is relatively swift, and most patients feel back to their normal selves within a week. If you have a desk job, you can usually return to work after one week, although it may take longer for more physically demanding occupations.

During the recovery period, it's important to avoid heavy lifting, intense gym workouts, and activities involving steam and saunas for about 6 weeks. Additionally, wearing a compression garment 24/7 for a minimum of 6 weeks is recommended.

In general, the stages of recovery follow a fairly predictable schedule, and significant deviations from this timeline are unlikely.

When the procedure is performed as a day case at the Cadogan Clinic, the recovery time tends to be reduced. This is due to the utilisation of advanced sedation techniques, which facilitate quicker initial recovery.

The goal of the surgery is to achieve a natural-looking result with breast implants. Therefore, the placement of the implant holds significance.

During your breast augmentation surgery consultation, your surgeon will assess various factors, including your existing anatomy and physique, whether you have plans for breastfeeding and the quantity and quality of your existing breast tissue. Based on this evaluation, the surgeon will determine the optimal position for the implant to ensure the best outcome.

Breast implants do not have a fixed expiration date. They typically do not need to be replaced unless they break or rupture, which is an extremely rare occurrence and is usually associated with the use of substandard implants or poor surgical techniques. While a rupture is not a serious health risk, it may result in a change in the shape or texture of the implant, necessitating its removal.

There are a few other reasons why breast implants may need to be replaced. Infection, which affects around 1-2% of cases, usually within the first 10 days of surgery, may require implant replacement. Capsule formation, observed in approximately 10% of individuals, involves the removal of a fibrous layer that forms around the implant, causing it to harden and feel unnatural. Implant rippling is another potential reason for replacement, often occurring in individuals with limited breast tissue and thin skin. Additionally, some patients may opt for implant replacement or removal for cosmetic reasons, such as desiring a larger, smaller, or more natural appearance.

After your procedure, it is advisable to limit physical activity to a minimum. Taking one to two weeks off work, refraining from driving for one week, and resuming restricted sexual activity after four weeks are recommended. Additionally, it is advisable to avoid exercise, including gym workouts, for a period of four to six weeks.

Maintaining the cleanliness of your surgical wounds is crucial. Our nursing team will provide detailed instructions on how to do this. It is important to avoid showering until one week after the surgery to minimise any potential risks.

Protecting your scars from sunlight is also vital for a period of six weeks following the surgery. After this timeframe, you may start moisturising and massaging your scars with a non-perfumed moisturiser. This routine helps soften and hydrate scar tissue, promoting improved healing.

Wearing a surgical garment for six weeks following augmentation is mandatory and should be strictly adhered to. This garment provides necessary support and aids in the healing process.

Breast enlargement surgery is a popular procedure and generally very safe. That said, as with all surgery, there are risks with breast enlargement surgery that you need to be aware of.

The best means of mitigating risks is to ensure that your surgeon is properly qualified and highly experienced in breast enlargement surgery, and we ensure this is the case with all of our patients by only choosing to work with the very best surgeons.
